address quickly and easily online. Please enter the information requested in the fields below.

|Description of Service / Conditions of eDelivery:|
By enrolling for eDelivery, you agree to receive online versions rather than paper copies of materials for your Prudential Financial life
insurance accounts. Materials available for eDelivery include semi-annual and annual reports, prospectuses, newsletters and any other
materials required to keep you informed about your account or product. You will be alerted when new documents are ready for viewing and be directed
to a website where you can view these materials. In addition, you will be able to view archived material for these products as it becomes available. Your
enrollment will last until you revoke it, or until we no longer offer the service.
|Option to receive hard copy|
If, after enrollment for eDelivery, you wish to receive printed material in addition to your email notification, you may request a hard copy delivery
from your broker or financial professional.
You may change your email address or cancel participation in eDelivery by going to Change Email Address or Cancel eDelivery and
following the instructions. If your email changes, you will need to provide us with a new email address by clicking the change option.
Enrollment for eDelivery requires that you have a personal computer with appropriate browser software, such as Internet Explorer,
Firefox, Chrome, and Safari for desktop and iPad or equivalent, and email software as well as communications access to the Internet.
This access may incur charges from Internet service providers and local telephone or cable companies. These costs are your responsibility.
In order to view PDF-formatted documents, you must have Adobe® Acrobat® viewer software. This software is
available for download free of charge.
